About the Role
As a Registered Nurse (RN) for our in-home senior care company in Ontario, you will play a critical role in delivering high-quality nursing care to elderly clients in the comfort of their own homes. You will work collaboratively with a multidisciplinary team to assess, plan, implement, and evaluate individualized care plans, ensuring the physical, emotional, and psychological well-being of our clients. Responsibilities:
Client Assessment: Conduct comprehensive health assessments of clients, including physical, cognitive, and psychosocial assessments, to identify their unique care needs and develop personalized care plans.
Medication Management: Administer medications as prescribed by healthcare professionals, ensuring accurate dosage, timing, and documentation. Monitor clients for potential adverse reactions and medication interactions, providing education and support as needed.
Wound Care: Perform wound assessments, dressings, and wound care procedures as prescribed, using evidence-based practices to promote wound healing and prevent complications.
Chronic Disease Management: Manage chronic health conditions, such as diabetes, hypertension, and heart disease, through education, monitoring, and support to optimize clients' health and well-being.
Pain Management: Assess and manage clients' pain using a holistic approach, incorporating pharmacological and non-pharmacological interventions to alleviate discomfort and improve quality of life.
Health Promotion and Education: Provide health education and counseling to clients and their families on topics such as disease prevention, medication adherence, nutrition, and lifestyle modifications to promote optimal health and wellness.
Care Coordination: Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to coordinate client care and services, facilitate referrals, and ensure continuity of care across healthcare settings.
Emergency Response: Respond promptly to medical emergencies and acute health crises, assessing and stabilizing clients' conditions, initiating appropriate interventions, and coordinating emergency medical services as needed.
Documentation: Maintain accurate and timely documentation of nursing assessments, interventions, and outcomes in client records, adhering to regulatory standards and best practices.
Quality Improvement: Participate in quality improvement initiatives, such as audits, performance improvement projects, and clinical practice guidelines, to enhance the quality and safety of care delivery.
Requirements
Current registration as a Registered Nurse (RN) with the College of Nurses of Ontario (CNO).
Bachelor of Science in Nursing (BScN) degree from an accredited institution; Master of Science in Nursing (MSN) preferred.
Minimum of 2 years of clinical nursing experience, preferably in community health, home care, or geriatrics.
Advanced knowledge and clinical skills in nursing assessment, medication management, wound care, and chronic disease management.
Excellent communication and interpersonal skills, with the ability to establish rapport and collaborate effectively with clients, families, and interdisciplinary team members.
Strong critical thinking and problem-solving abilities, with the capacity to make sound clinical judgments and prioritize care in complex situations.
Proficiency in electronic health record (EHR) systems and other healthcare technology tools for documentation and communication.
Commitment to professional development and lifelong learning, staying abreast of current best practices, guidelines, and evidence-based interventions in nursing.
Compassionate and patient-centered approach to care, with a genuine passion for supporting seniors' health and well-being in a home care setting.
Valid driver's license and access to a reliable vehicle for travel to clients' homes within the designated service area.
